PNEUMONIA ASPIRASI Definisi Pneumonia aspirasi merupakan peradangan yang mengenai parenkim paru, distal dari bronkus terminalis yang mencakup bronkiolus respiratorius, dan alveoli, serta menimbulkan konsolidasi jaringan paru dan gangguan pertukaran gas setempat yang disebabkan oleh aspirasi benda asing baik yang berasal dari dalam tubuh maupun di luar tubuh penderita. 1 Epidemiologi Di Amerika pneumonia aspirasi yang terjadi pada komunitas (PAK) adalah sebanyak 1200 per 100.000 penduduk per tahun, sedangkan pneumonia aspirasi nosokomial (PAN) sebesar 800 pasien per 100.000 pasien rawat inap per tahun. PA lebih sering dijumpai pada pria daripada perempuan, terutama usia anak atau lanjut. PNEUMONIA ASPIRASI
Definisi
Pneumonia aspirasi merupakan peradangan yang mengenai parenkim paru,

Epidemiologi
Di Amerika pneumonia aspirasi yang terjadi pada komunitas (PAK) adalah
sebanyak 1200 per 100.000 penduduk per tahun, sedangkan pneumonia aspirasi nosokomial
(PAN) sebesar 800 pasien per 100.000 pasien rawat inap per tahun. PA lebih sering dijumpai

Etiologi
Terdapat 3 macam penyebab sindroma pneumonia aspirasi, yaitu aspirasi
asam lambung yang menyebabkan pneumonia kimiawi, aspirasi bakteri dari oral dan
oropharingeal menyebabkan pneumonia bakterial, Aspirasi minyak, seperti mineral oil atau
vegetable oil dapat menyebabkan exogenous lipoid pneumonia. Apirasi benda asing
merupakan kegawatdaruratan paru dan pada beberapa kasus merupakan faktor predisposisi
pneumonia bakterial.1,3
Infeksi terjadi secara endogen oleh kuman orofaring yang biasanya
polimikrobial namun jenisnya tergantung kepada lokasi, tempat terjadinya, yaitu di
komunitas atau di RS. Pada PAK, kuman patogen terutama berupa kuman anaerob obligat
(41-46%) yang terdapat di sekitar gigi dan dikeluarkan melalui ludah, misalnya Peptococcus
yang juga dapat disertai Klebsiella pnemoniae dan Stafilococcus, atau fusobacterium
nucleatum, Bacteriodes melaninogenicus, dan Peptostreptococcus. Pada PAN pasien di RS
kumannya berasal dari kolonisasi kuman anaerob fakultatif, batang Gram negatif,
pseudomonas, proteus, serratia, dan S. aureus di samping bisa juga disertai oleh kuman
ananerob obligat di atas.1,4
Daya tahan traktus respiratorius
Mekanisme daya tahan traktus respiratorius bagian bawah sangat efisien untuk mencegah
infeksi dan terdiri dari:3
a. Susunan anatomis rongga hidung
b. Jaringan limfoid di nasoorofaring
c. Bulu getar yang meliputi sebagian besar epitel traktus respiratorius dan sekret yang
dikeluarkan oleh set epitel tersebut
d. Refleks batuk
e. Refleks epiglotis yang mencegah terjadinya aspirasi sekret yang terinfeksi.
f. Drainase sistem limfatik dan fungsi menyaring kelenjar limfe regional.
g. Fagositosis, aksi enzimatik dan respons imuno-humoral terutama dari
imunoglobulin A (IgA).

Patofisiologi
Aspirasi merupakan hal yang dapat terjadi pada setiap orang. Di sini terdapat peranan
aksi mukosilier dan makrofag alveoler dalam pembersihan material yang teraspirasi. Terdapat
3 faktor determinan yang berperan dalam pneumonia aspirasi, yaitu sifat material yang
teraspirasi, volume aspirasi, serta faktor defensif host.2
Perubahan patologis pada saluran napas pada umumnya tidak dapat dibedakan antara
berbagai penyebab pneumonia, hampir semua kasus gangguan terjadi pada parenkim disertai
bronkiolitis dan gangguan interstisial. Perubahan patologis meliputi kerusakan epitel,
pembentukan mukus dan akhirnya terjadi penyumbatan bronkus. Selanjutnya terjadi infiltrasi
sel radang peribronkial (peribronkiolitis) dan terjadi infeksi baik pada jaringan interstisial,
duktus alveolaris maupun dinding alveolus, dapat pula disertai pembentukan membran hialin
dan perdarahan intra alveolar. Gangguan paru dapat berupa restriksi, difusi dan perfusi.2
Pneumonia aspirasi mengarah kepada konsekuensi patologis akibat secret
orofaringeal, nanah, atau isi lambung yang masuk ke saluran napas bagian bawah.
Kebanyakan individu mengaspirasi sedikit secret orofaringeal selama tidur, dan secret
anaerob, misalnya aminoglikosida dikombinasikan dengan sefalosporin generasi ke
3 atau 4, atau klindamisin. Perlu dipertimbangkan pola dan resistensi kuman di
rumah sakit bersangkutan. Dilakukan evaluasi hasil terapi dan resolusi terhadap
terapi berdasarkan gambaran klinis bakteriologis untuk memutuskan penggantian
atau penyesuaian antibiotic (AB).1
Tidak ada patokan pasti lamanya terapi. Antibiotik perlu diteruskan hingga kondisi
pasien baik, gambaran radiologis bersih atau stabil selama 2 minggu. Biasanya
diperlukan terapi 3-6 minggu. 1
Follow up
Pasien dengan keadaan hemodinamik berat atau dengan distress respiratory
di rawat di ICU.
Pasien dengan respiratori yang stabil di rawat di bangsal perawatan umum.

To drain mucus from the upper lobe apical segments, the patient sits in a comfortable position on a bed or flat surface and leans on a pillow against the headboard of the bed or the caregiver. The caregiver percusses and vibrates over the muscular area between the collar bone and very top of the shoulder blades (shaded areas of the diagram) on both sides for 3 to 5 minutes. Encourage the patient to take a deep breath and cough during percussion in order to help clear the airways. Do not percuss over bare skin.
The patient sits comfortably in a chair or the side of the bed and leans over, arms dangling, against a pillow. The caregiver percusses and vibrates with both hands over upper back on both the right and left sides.
In position #3, the patient lies flat on the bed or table with a pillow for comfort under his or her head and legs. The caregiver percusses and vibrates the right and left sides of the front of the chest, between the collar bone and nipple.
The patient lies with their head down toward the foot of the bed on the right side, hips and legs up on pillows. The body should be rotated about a quarter-turn towards the back. A pillow can also be placed behind the patient and their legs slightly bent with another pillow between the knees. The caregiver percusses and vibrates just outside the nipple area.
The patient lies head-down on his left side, a quarter-turn toward the back with the right arm up and out of the way. The legs and hips should be elevated as high as possible. A pillow may be placed in back of the patient and between slightly bent legs. The caregiver percusses and vibrates just outside the right nipple area.
The patient lies on his right side with his head facing the foot of the bed and a pillow behind his back. The hips and legs should be elevated as high as possible on pillows. The knees should be slightly bent and a pillow should be placed between them for comfort.
The caregiver percusses and vibrates over the lower ribs on the left side, as shown in the shaded part of the diagram. This should then be repeated on the opposite side, with percussion and vibration over the lower ribs on the right side of the chest.
The patient lies on his right side with his head facing the foot of the bed and a pillow behind his back. The hips and legs should be elevated as high as possible on pillows. The knees should be slightly bent and a pillow should be placed between them for comfort.
The caregiver percusses and vibrates over the lower ribs on the left side, as shown in the shaded part of the diagram. This should then be repeated on the opposite side, with percussion and vibration over the lower ribs on the right side of the chest.
The patients lies on his or her stomach, with the hips and legs elevated by pillows. The caregiver percusses and vibrates at the lower part of the back, over the left and right sides of the spine, careful to avoid the spine and lower ribs.
The patient lies on his right side, leaning forward about one-quarter of a turn with hips and legs elevated on pillows. The top leg may be flexed over a pillow for support and comfort.
The caregiver percusses and vibrates over the uppermost portion of the lower part of the left ribs, as shown in the shaded area. This should then be repeated on the opposite side, with percussion and vibration over the uppermost portion of the right side of the lower ribs.
For this position, the patient lies on his stomach on a flat bed or table. Two pillows should be placed under the hips.
The caregiver percusses and vibrates over the bottom part of the shoulder blades, on both the right and left sides of the spine, avoiding direct percussion or vibration over the spine itself.
