Adam Malik Medan PENDAHULUAN Penemuan obat antiretroviral (AV) pada tahun 1996 mendorong suatu revolusi dalam perawatan pasien HIV di negara maju. Meskipun belum mampu menyembuhkan penyakit dan menambah tantangan dalam efek samping serta resistensi kronis terhadab obat, namun secara dramatis terapi ARV menurunkan angka kematian dan kesakitan, meningkatkan kualitas hidup pasien dengan HIV, dan meningkatkan harapan masyarakat, sehingga pada saat ini HIV dan AIDS telah diterima sebagai penyakit yang dapat dikendalikan dan tidak lagi dianggap sebagai penyakit yang menakutkan.(1) Obat ARV terdiri dari beberapa golongan seperti nucleoside reverse transciptase inhibitor, nucleotide reverse transciptase inhibitor, non-nucleoside reverse transciptase inhibitor, dan inhibitor protease.(2) Baku pengobatan terdiri dari gabungan beberapa golongan obat, dan biasanya terdiri dari tiga obat atau disebut triple therapy dan kombinasi golongan obat HIV ini biasanya juga disebut dengan Highly Active Antiretrovial Therapy (HAART). WHO merekomendasikan bahwa rejimen lini pertama adalah 2 NRTI ditambah satu obat NNRTI.(3) NNRTI adalah golongan obat yang mengganggu proses penciptaan DNA virus dari RNA, dengan mengikat enzim reverse traskiptase dan menghalangi keiatannya. Bebepara penelitian menunjukan kepatuhan yang baik (>95%) terhadap rejimen antiretroviral dibutuhkan untuk mencapai supresi jangka panjang dari replikasi virus. Hal ini juga diketahui bahwa ketidakpatuhan menghasilkan resistensi virus terhadap obat dan kegagalan dalam pengobatan. Kepatuhan berbanding terbalik dengan jumlah dosis perhari. Dalam rangka upaya untuk menyederhanakan rejimen dan meningkatkan kepatuhan, maka dosis sekali sehari merupakan pilihan yang paling diinginkan oleh pasien yang terinfeksi HIV. Semakin banyak obat dari beberapa kelas dilisensikan untuk dosis sekali sehari (misalnya, tenefovir, evafirenz dan atazanavir. PENDAHULUAN
Penemuan obat antiretroviral (AV) pada tahun 1996 mendorong suatu revolusi dalam
perawatan pasien HIV di negara maju. Meskipun belum mampu menyembuhkan penyakit dan
menambah tantangan dalam efek samping serta resistensi kronis terhadab obat, namun secara
dramatis terapi ARV menurunkan angka kematian dan kesakitan, meningkatkan kualitas hidup
pasien dengan HIV, dan meningkatkan harapan masyarakat, sehingga pada saat ini HIV dan AIDS
telah diterima sebagai penyakit yang dapat dikendalikan dan tidak lagi dianggap sebagai penyakit
yang menakutkan.(1)
Obat ARV terdiri dari beberapa golongan seperti nucleoside reverse transciptase inhibitor,
nucleotide reverse transciptase inhibitor, non-nucleoside reverse transciptase inhibitor, dan
inhibitor protease.(2) Baku pengobatan terdiri dari gabungan beberapa golongan obat, dan biasanya
terdiri dari tiga obat atau disebut triple therapy dan kombinasi golongan obat HIV ini biasanya juga
disebut dengan Highly Active Antiretrovial Therapy (HAART). WHO merekomendasikan bahwa
rejimen lini pertama adalah 2 NRTI ditambah satu obat NNRTI.(3) NNRTI adalah golongan obat
yang mengganggu proses penciptaan DNA virus dari RNA, dengan mengikat enzim reverse
traskiptase dan menghalangi keiatannya.
Bebepara penelitian menunjukan kepatuhan yang baik (>95%) terhadap rejimen
antiretroviral dibutuhkan untuk mencapai supresi jangka panjang dari replikasi virus. Hal ini juga

tambahan, kelayakan dosis sekali sehari pada obat yang berlisensi untuk dosis dua kali sehari
sedang di eksplorasi, salah satunya adalah nevirapine.(4)
NEVIRAPINE
Nevirapine (NVP) adalah suatu non-nucleoside reverse transciptase inhibitor (NNRTI)
untuk HIV-1. Hal ini menghambat replikasi HIV-1 dengan mengikat langsung transkiptse-balik
(reverse transciptase RT) dan memblokir sentrum katalisis enzim. Setelah NVP berikatan dengan
RT, akan menyebabkan perubahan konformasi yang menginaktivasi enzim, sehingga mencegah
polimerisasi RNA virus ke DNA. NVP sangat spesifik untuk RT HIV-1 dan tidak mengganggu
polimerasi DNA manusia. NVP dengan bebas memasuki sel dan aktif dalam banyak sel yang

FARMAKOKINETIK
Distribusi
NVP bersifat lipofilik dan tidak terionisasi pada pH fisiologis. Setelah pemberian secara
intravena pada pasien dewasa, volume distribusi (Vdss) dari NVP adalahh 1.21 ± 0.09L/l=kg,
menunjukkan bahwa NVP mudah didistribusikan pada manusia. Sebuah peninjauan dari Pacifici
menunjukkan bahwa pada neonatus volume distibusinya lebih besar dibandingkan dengan dewasa.

MATERIALS AND METHODSThe participants were HIV-1-infected persons with abso-
lute CD4 counts of <400/,u. The volunteers were 18 men and3 women ranging in age from 19 to 46 years. Six volunteershad AIDS, and 10 had AIDS-related complex; their CD4counts ranged from 0 to 379 (mean + standard deviation, 191± 124). By transmission category, there were 12 homosexualmales, 2 homosexual males who also used intravenousdrugs, and 7 intravenous drug users. Three individuals wereLatino and one was African-American. The University ofMassachusetts Medical School Human Subjects Committeereviewed and approved the study and the document whichall participants signed to provide informed consent.Three persons receiving each dose level were studied
simultaneously. Doses were 2.5, 12.5, 25, 50, 100, 200, and400 mg given as tablets containing 2.5, 12.5, or 50 mg ofnevirapine. Table 1 shows the doses achieved in terms ofmilligrams per kilogram for individuals in each cohort.Concomitant medications were kept to a minimum, anddrugs with known interactions with phenobarbital wererequired to be stopped 28 days prior to the trial, as were anyinvestigational agents. Zidovudine was continued until 7days before the trial and was resumed upon its completion.A urine specimen for drug screening was collected at thetime of admission to hospital for the study. Participantsfasted overnight under observation and received a singledose of nevirapine after initial blood sampling the followingmorning.Blood for determination of nevirapine levels in plasma was
drawn at 0.25, 0.5, 0.75, 1, 1.5, 2, 3, 4, 6, 8, 12, 16, 24, 28,and 32 h after dosing. An electrocardiograph was obtained at2 h postdosing. Volunteers were discharged from the hospi-tal after the 32-h blood draw. Blood for repeat determinationof nevirapine levels in plasma were drawn at 48 and 72 h,since preclinical studies suggested a complete plasma nevi-rapine concentration-time curve could be obtained withinthis time frame. After administration of the 12.5-mg dose, itbecame apparent that nevirapine clearance in humans waslower than that predicted from studies in animals, so addi-tional plasma samples were collected at 96 h and between168 and 200 h when possible.
Plasma was separated promptly, stored at -80° C, andassayed by high-pressure liquid chromatography (HPLC)after inactivating the virus by heating at 56° C for 30 min. Theassay was validated for precision and accuracy and is linearfrom 25 to 10,000 ng/ml (5). Extraction recovery was greaterthan 88% of the spiked amounts at all concentrations forboth nevirapine and the internal standard. Extracted drug-free human plasma samples demonstrated the absence of anyinterfering or coeluting components. A total of 25 standardcurves were used during analysis of the study samples. Thecoefficient of variation of the slopes of the curves as deter-mined from unweighted linear regression was 3.4%. Sixdifferent concentrations of quality control standards runduring the study had intraday and interday relative standarddeviations ranging from 4.6 to 12.4%, and the deviation fromthe nominal value was 9.7% or less. The limit of quantita-tion, defined as the lowest standard with a relative standarddeviation of <15%, was 25 ng/ml.The area under the plasma concentration-time curve
(AUC) was calculated by the linear trapezoidal rule method.The relationships of AUC and the peak concentration ofnevirapine in plasma (Cm.) with dose were examined bylinear and quadratic least-squares regression of the AUC orCmaxdata versus dose. The quadratic term was required tobe statistically significant at the 0.05 level to be accepted asevidence of deviation from linearity.Apparent oral clearance was estimated from the ratio of
the dose administered to the observed AUC. The apparentvolume of distribution at steady state was calculated fromthe ratio of dose-adjusted area under the moment curve toAUC. t1/2 was estimated by linear regression of log plasmanevirapine concentration-versus-time data. The datumpoints included in the regression analysis of the terminalelimination phase were determined from visual inspection ofgraphic semilogarithmic plots of the data. Steady-stateplasma nevirapine concentrations, assuming once-daily dos-ing, were calculated for each individual from the ratio ofAUC to the dosing interval. Trough concentrations pre-dicted to be achieved at steady state were estimated from theindividual average steady-state concentrations, individualt112s, and the dosing interval chosen for initial multiple-dosestudies (24 h) (3).
RESULTSPharmacokinetics. Individual semilogarithmic plots of
plasma nevirapine concentration-time course data after ad-ministration of single doses of 12.5, 200, and 400 mg ofnevirapine are shown in Fig. 2A to C, respectively. The firsttime point used in the linear regression for calculation of thet112 for each individual is shown by the dashed line. For themajority of the subjects, six or nine datum points wereincluded in the regression analysis. Only four datum pointswere available for estimation of the t112 of nevirapine insubject 16 (200-mg dose level). All subjects had detectablelevels of nevirapine in plasma after administration of thelowest (2.5-mg) dose, but the majority of the concentrationswere below the linear range of the assay, precluding furtherpharmacokinetic analysis. Concentrations in the plasma ofall other subjects at all time points in the study were abovethe limit of quantitation of the assay.Noncompartmental pharmacokinetic parameters are listed
for individual subjects in Table 1. The drug was rapidlyabsorbed; substantial amounts of nevirapine were present inthe systemic circulation as early as 15 min after the tabletswere ingested. The time-concentration profiles observed

diketahui sampai sejauh mana kadar obat CSF berkolerasi dengan kadar obat jaringan otak.
Berkurangnya viral load CSF telah dibuktikan pada pasien yang memakai rejimen yang berdasarkan
nevirapine.(5)
Farmakokinetik Plasma
NVP mudah diserap (>90%) setelah pemberian oral pada sukarelawan yang sehat dan pada
orang dewasa yang terinfeksi HIV-1. Bioavibilitas absolut pada 12 orang dewasa yang sehat setelah
pemberian dosis tunggal adalah 90%, baik diberikan sebagai tablet atau larutan oral. Pemusatan
data dari beberapa penelitian menunjukkan bahwa konsentrasi plasma puncak dari 2 ± 0.4mg/mL
(7.5mM) dicapai dalam waktu 4 jam setelah pemberian nevirapine dosis oral tunggal 200mg.
Setelah beberapa dosis, puncak konsentrasi NVP tampak meningkat pada rentang dosis
200-400mg/hari. Kadar optimal konsentrasi terendah NVP sebesar 4.5 ± 1.9mg/mL (atau 17 ±
7nM) yang dicapai pada 400mg/day. Parameter ini tidak berubah secara substansial apakah obat itu
diberikan dengan atau tanpa makanan atau dengan buffer basa (seperti antasida).(5)
Metabolisme/Eliminasi
Nevirapine secara luas dimetabolisme melalui sitokrom p450 (CYP450) menjadi bebrapa
hydroksilasi metabolit in vivo dan in vitro. penelitian In vitro pada mikrosom hati manusia
menunjukkan bahwa metabolisme oksidatif dari NVP di mediasi terutama oleh CYP450 isozim dari
famili CYP3A, meskipun isozim lainnya juga memainkan peranan sekunder. Metabolisme CYP450,
konjugasi glukoronida, dan ekskresi melalui urine dari metabolit glukuronidasi merupakan jalur
utama dari metabolisme nevirapine dan eliminasi pada manusia.(5, 6)
Nevirapine juga telah terbukti sebagai penginduksi enzim metabolik CYP450. Autoinduksi
juga menghasilkan perubahan korespon dalam l paruh-waktu fase terminal dari NVP dalam plasma
dari ∼45 jam (dosis tunggal) menjadi ∼25-30 jam setelah dosis ganda. Farmakokinetik dari NVP
pada pasien dengan gangguan ginjal tidak berubah. Selama dialisis NVP dibersihkan dari badan dan
dosis pengulangan setelah dialisi harus dipertimbangkan.; pasien dengan gangguan hati sedang
maupun berat (skor Child-Pugh > 8 asites) beresiko untuk menumpuknya NVP, dan yang lebih
penting, NVP menimbulkan toksisitas hati. Dalam keadaan itu penggunaan NVP tidak dianjurkan.
Jika NVP sangat diperlukan dalam kadaan seperti itu, pengurangan dosis harus dipertimbangkan.(5)
Penelitian oleh Hong-Brown et al juga menunjukkan penurunan sintesis protein dalam
miosit oleh karena keberadaan NVP dalam sel ini. Penurunan sintesa protein berkolerasi dengan
penurunan tingkat fosforilasi dari translasi yang menginisiasi protein.(7)

Jenis Kelamin/Ras/Usia
Tidak ada perbedaan yang cukup besar terhadap jenis kelamin atau ras yang dilaporkan
dalam beberapa uji klinis. Di dalam satu penelitian fase I melibatkan sukarelawan yang sehat (15
wanita, 15 pria), berat badan-disesuaikan distribusi volume (Vdss) dari NVP lebih tinggi pada
wanita (1.54L/kg) dibandingkan dengan pria (1.38L/kg), menunjukkan bahwa NVP di distribusikan
lebih banyak pada wanita. Namun, perbedaan ini diimbangi dengan waktu paruh fase terminal lebih
pendek pada wanita. Sehingga tidak ada perbedaan jenis kelamin yang signifikan dalam konsentrasi
NVP setelah pemberian baik dosis tunggal ataupun dosis ganda.(5, 8) Pada orang dewasa,
farmakokinetik NVP tidak berubah jauh pada usia (berkisar18-68 tahun). Tingkat pembersihan yang
nyata pada anak-anak mencapai maksimum pada umur 1-2 tahun, dan kemudian menurun pada
waktu kewaktu. akibatnya dosis yang dianjurkan untuk anak 2 bulan sampai 8 tahun adalah 4mg/kg
sekali sehari untuk 14 hari pertama setelah itu diikuti dengan 7 mg/kg dua kali sehari. dibandingkan
dengan 4mg/kg sekali sehari untuk 14 hari pertama diikuti dengan 4mg/kg dua kali sehari pada
anak-aanak usia 8 tahun dan lebih tua.(5)
Studi Dosis Manusia
Dosis harian 12.50, 50, 200, dan 400 mg diteliti pada pasien dewasa dengan hitungan CD4+
T-limfosit <400 cell/mm3, dosis-efek yang proposional ditemukan, dengan dosis 400mg/hari
menjadi unggul dalam hal besar dan durasi efek.(5, 8, 9) Dalam studi yang terpisah dosis 600mg
berikatan dengan meningkatnya toksisitas dan tidak ada keuntungan yang besar pada aktifitas
antiretrovirus. Batasan dosis toksisitas yang paling utama adalah ruam. Selain itu, terjadi juga
peningkatan enzim hati dan sesekali mengantuk. Akibatnya, dosis 400mg sehari dipilih untuk
pengembangan klinis.
TOKSISITAS
Efek samping obat yang paling sering dilaporkan sehubungan dengan pengobatan dengan
NVP pada uji klinis termasuk ruam, demam, lemah, sakit kepala, mengantuk, mual, peningkatan
enzim hati dan heptitis kimia. Ruam adalah efek samping yang paling umum. yang dikatikan
dengan penggunaa NVP. Dalam empat percobaan terkontrol, kombinasi terapi dengan NVP,
kejadian keseluruhan ruam, terlepas dari hubungan sebab akibat yang ditetapkan oleh peneliti ,
adalah 35% pada kelompok NVP dan 19% pada kelompok kontrol. Perbedaan ini bermakna secara
statistik, untuk kejadian secara keseluruhan 16% dari ruam disebabkan oleh NVP.(5) Ruam yang
diakibatkan oleh NVP biasanya ringan dan sembuh dengan sendirinya, dan resiko unttuk ruam
sangat tinggi dalam 6 minggu pertama terapi. Diantara pasien yang di terapi dengan NVP pada

empat uji klinis terkontrol, total 6.5% mengalami ruam tingkat 3 dan 4, dibandingkan dengan 1.3%
pada kontrol.(10) Hanya 7% pasien pada grup NVP diberhentikan karena ruam, dibandingkan 1 %
pada grup kontrol, dan kebanyakan dari mereka mengalami ruam tingkat 3 atau 4. Berdasarkan
2861 pasien yang di terapi dengan NVP dalam berbagai uji klinis, sembilan pasien dinyatakan
Sindroma Steven-Jhonson (SJS), termasuk dua yang dilaporkan mengalami SJS/Toxic Epidermal
Necrolysis (TEN). Ini menggambarkan insiden SJS keseluruhan 0.3%. Meski kasus fatal telah
dilaporkan badan pengawas setelah pemasaran, kebanyakan kasus dikelola dengan terapi suportif,
termasuk penggantian cairan dan kontrol nyeri. Insiden ruam pada anak-anak tidak berbeda dengan
dewasa.(5, 10)
Sampai saat ini, tidak ada faktor telah diidentifikasikan yang menjadi predisposisi seorang
pasien akan mendapatkan ruam. Juga, mekanisme dari NVP yang menyebabkan ruam masi belum
diketahui pasti. Konsentrasi plasma dari obat tidak berkolerasi dengan terjadinya ruam, ataupun
jenis kelamin, ras, obat-obat yang bersamaan, riwayat ruam, ataupun stadium penyakit. Dosis
rejimen NVP yang saat ini dianjurkan adalah 200mg sehari sekali selama 2 minggu, selanjutnya
200mg 2 kali sehari. Berdasarkan analisa retrospektif menunjukkan rejimen ini lebih jarang
dikaitkan dengan ruam. Namun demikian pasien harus diperingatkan dengan hati-hati bahwa efek
obat ini dapat berpotensi serius. Dalam program penggunaan NVP di Belanda, tidak ada muncul
ruam pada pasien yang sebelumnya dirawat dengan antiretroviral yang lain dan mempunyai kadar
HIV-1 plasma yang tak terdeteksi.(5, 11) Tabel 1 memberikan pedoman penatalaksanaan empiris
ruam yang dikembangkan dari para ahli. Pedoman ini memberikan arahan untuk melanjutkan atau
menghentikan pemakaian NVP pada pasien yang mengalami ruam. Kortikosteroid tidak mencegah

cases are managed with supportive therapy, including fl uid replacement and pain control. The incidence of rash in pediat-ric patients is no different from that in adults.27
To date, no factors have been identifi ed that might predis-pose a patient to the development of rash. Also, the mechanism of NVP-induced rashes remains to be determined. Plasma con-centrations of the drug do not correlate with the occurrence of rash, nor has a proven relationship been identifi ed with gender, race, concomitant medications, rash history, or disease stage. The currently recommended NVP dosing regimen is 200 mg qd for 2 weeks, followed by 200 mg bid thereafter. This is based on retrospective analyses suggesting that this regimen is less frequently associated with rash. Nevertheless, patients need to be carefully warned about this potentially serious drug-related effect. In a NVP compassionate use program in the Netherlands, no rashes occurred in patients who had been pre-treated with other antiretroviral agents and had plasma HIV-1 levels below the limit of detection.30 Table 13-1 provides a set of empiric rash management guidelines developed with input from an expert panel. These guidelines provide appropriate direction for the continued use or discontinuation of NVP in the event that a patient experiences rash. A number of proto-cols for rash prevention, including the use of corticosteroids, antihistamines, and slower dose escalation, have been inves-tigated. Corticosteroids do not appear to prevent NVP-related rash, and may even cause an increase in its incidence.31,32,34
The roles of antihistamines or slower dose escalation for the prevention of NVP-related rash require further evaluation in prospective controlled trials. Whether corticosteroids or anti-histamines may play a role in the treatment of NVP-related rash is also dependent on further investigations.36
Elevations of liver enzymes, including hepatitis, have also been reported in NVP-treated patients. Isolated gamma-glutamyl transpeptidase increases are relatively frequent; however, these are of no clinical consequence and have been attributed to liver enzyme autoinduction of NVP metabolism. Drug-induced hepatitis was reported in nine of 906 (1%) patients chronically treated with NVP in controlled clinical tri-als.27 A similar result was obtained in the BI 1090 trial.35 This was a randomized, placebo-controlled study comparing 1121 patients taking NVP plus two NRTIs with 1128 patients tak-ing placebo plus two NRTIs over 2 years. Using an expanded hepatitis defi nition (including viral hepatitis), 2.8% of patients in the NVP group and 1.4% in the placebo group had hepatitis over the course of the study (P ! 0.026). No signifi cant differ-ence in the incidence of serious hepatic events was observed between the two groups. The overall rate of withdrawal as a result of hepatitis in adult NVP trials has been 0.8%, but a lim-ited number of fatal hepatitis cases associated with NVP use have been reported. Data which have aroused some concern have been reported from the blinded FTC-302 study,37 a phase III study comparing the NRTIs emtricitabine and lamivudine, in which all patients also received stavudine and were strati-fi ed to receive either NVP or EFV, according to their baseline viral load. A total of 87% of the study subjects were black, and 59% were women. There was a high incidence of grade 3 and 4 hepatotoxicity, affecting 58 (15%) of 468 subjects, all of whom were receiving NVP, and two cases were fatal. The vast majority of cases occurred within the fi rst 8 weeks of therapy, with a temporal association with the dose escalation of NVP from 200 to 400 mg at day 14 of treatment. Peran antihistamin atau eskalasi dosis lambat untuk pencegahan ruam yang berkaitan dengan NVP
memerlukan penelitian lebih lanjut secara prospektif terkontrol. Apakah kortikosteroid atau
antihistamin mempunyai peranan dalam pengobatan ruam yang berkaitan dengan NVP juga

PENYEDERHANAAN DOSIS

Appropriate Use of Nevirapinefor Long-Term Therapy
To the Editor—The recent article bySanne et al. [1] described possible risk fac-tors associated with hepatic adverse eventsthat can occur while nevirapine is beingtaken in a clinical trial. We are concernedabout several key issues either overlookedor minimized in the article. It is impor-tant to understand that the prelimi-nary results of the FTC-302 study pro-vided the impetus for Boehringer Ingel-heim (BI) to conduct a comprehensiveanalysis of hepatotoxicity in all large BIcontrolled and uncontrolled studies. Thisanalysis resulted in warnings that femalesex and higher CD4 cell count at the in-itiation of therapy increases the risk ofhepatotoxicity, particularly during thefirst 6 weeks of treatment. This infor-mation has been widely communicatedthrough changes to the prescribing in-formation and through BI communica-tions to physicians. Most recently, BI hasrecommended against the initiation ofnevirapine treatment in women withCD4 cell counts 1250 cells/mm3 or inmen with CD4 cell counts 1400 cells/mm3 unless the benefit outweighs therisk (see http://www.fda.gov/cder/drug/advisory/Nevirapine.htm). As shown infigure 1, there is a clear demarcation inthe risk of developing nevirapine-asso-ciated symptomatic hepatitis at the CD4cell count cutoffs mentioned above.
In our comprehensive analyses, the fre-quency of patients with symptomatic he-patic events in the lower CD4 cell countgroups (women with counts !250 cells/mm3 and men with counts !400 cells/mm3) was significantly reduced and wasconsistent with frequencies found in as-sociation with other antiretrovirals, in-cluding efavirenz (1%–2%). We are con-cerned that, because study FTC-302
excluded patients with CD4 cell counts!200 cells/mm3, the major predictive fac-tor of hepatic events was removed fromconsideration, therein biasing the au-thors’ conclusion that a low body massindex (BMI) is predictive of nevirapinehepatotoxicity.
Additionally, there was only a smallnumber of patients with BMIs !18.5(4.4% of total), and these accounted foronly a small proportion of patients withhepatotoxicity (11.3%). This result sug-gests that there were other explanationsfor the high rate of hepatic events ob-served in this study, including the meanCD4 cell count of 406 cells/mm3 in pa-tients with early events and coadministra-tion of other potentially hepatotoxic med-ications, including stavudine. Thus, theconclusion that BMI is predictive of ne-virapine-associated hepatotoxicity maybe spurious, because of a combination ofstudy design, statistical artifact, and theinvolvement of other factors that werenot assessed in the article.
It is also important to emphasize that
patients were not monitored for the ap-pearance of rash or hepatotoxicity untilafter 4 weeks of treatment. This is insuf-ficient, according to current recommen-dations, especially given that the majorityof these events occur during the first 6weeks of treatment. Likewise, promptdiscontinuation of nevirapine is recom-mended after the onset of grade 3 or 4rash or constitutional symptoms sug-gestive of hepatitis. It is possible that rec-ommended monitoring and more-rap-id discontinuation of trial medicationswould have lessened the severity of re-actions in some patients in FTC-302.
Although BI has initiated a toxicoge-nomics program to study the complex re-lationship between nevirapine and hyper-sensitivity reactions, we believe that, bymaking the relatively simple recommen-dation based on CD4 cell count, there willbe a significant reduction in the incidenceof nevirapine-associated adverse events.By working to improve nevirapine safetywithin the standard guidelines for treat-ment initiation, an important option for

diberikan dosis dua kali sehari, ini masih jauh diatas IC50. Penelitian SCAN menemukan bahwa
NVP sehari sekali adalah efektif dan ditoleransi dengan baik sama seperti NVP dua kali sehari pada
infeksi HIV tahap awal.(18) Demikian pula, penelitian Atlantic menunjukkan bahwa tiga obat
kombinasi termasuk dua nukleosida (didianosin dan stavudine) ditambah NVP dosis sekali sehari,
mempunyai efek antivirus dan CD+4 serupa dengan rejimen kombinasi termasuk dua nukleosida
yang sama dan satu penghambat protease (idinavir).(19)
Dari penelitian Calmy A et al. menyatakan inisiasi dosis NVP sekali sehari pada pasien yang
telah tersupresi replikasi HIV-1nya, dan juga pasien yang beralih dari dosis NVP dua kali sehari
yang telah stabil ke rejimen sekali sehari, memiliki profile yang baik (Tablel 2).(20)
Negredo et al. meneliti keamanan dan kemanjuran pasien beralih dari berbagai rejimen dua
kali sehari (dengan atau tanpa NVP) menjadi satu kali sehari nevirapin 400mg, dianosine 400mg
dan tenofovir 300mg. (21) Total pasien 196, 85 dari mereka beralih ke rejimen sehari sekali. Setelah
48 minggu, 65 dari 85 pasien (76%) pada grup sehari sekali dipertahankan plasma viral load
dibawah 50 copies/mL, dibandingkan dengan 72 dari 84% (86%) dalam grup yang masih
melanjutkan dosis dua kali sehari (P=0.176). Efek samping yang menyebabkan pemberhentian
pengobatan lebih banyak pada group sehari sekali. Tiga subjek (3.6%) yang melanjutkan dosis dua
kali sehari diberhentikan terapinya dibandingkan dengan 12 subjek (14.1%) yang beralih ke dosis
sehari sekali. Setelah 48 minggu. dalam grup dosis sehari sekali dilaporkan kepatuhan sebesar 97%
dibandingkan dengan grup dua kali sehari sebesar 69.2%. Data ini menunjukkan bahwa manfaat
dari peningkatan kepatuhan lebih dibandingkan dengan efek samping, dan dalam penelitian ini,
daily groups (1.1% and 1.4%, respectively; P=0.47). HSR consisted of skin rash in 210 (3.7%) patients, and occurred significantly more frequently in patients tak-ing NVP once daily (twice daily 3.5% and once daily 5.0%; P=0.041). Multivariate logistic regression with discontinuation because of HSR as the outcome variable showed a statistically significant interaction between the NVP dose and HIV-1 RNA at the start of NVP. There-fore, patients were classified into four groups on the basis of NVP dose and HIV-1 RNA at the start of NVP. The group of patients using NVP dosed twice daily and hav-ing detectable HIV-1 RNA served as the reference group. The final model (Table 2) showed that, compared with patients with detectable HIV-1 RNA using NVP dosed
twice daily, there was a statistically significantly higher risk of NVP discontinuation in patients with detectable HIV-1 RNA starting NVP once daily (detectable once daily versus detectable twice daily odds ratio [OR] 1.52; P=0.04). However, the risk was actually significantly lower in patients with undetectable HIV-1 RNA starting NVP once daily (undetectable once daily versus detect-able twice daily OR 0.43; P=0.04). The protective effect of having undetectable HIV-1 RNA was also present in patients using NVP dosed twice daily (undetectable twice daily versus detectable twice daily OR 0.67; P=0.02).
Among the 734 patients who, after using a 200 mg twice-daily NVP-based regimen for some time, switched to a 400 mg once-daily regimen, only 3 (0.41%) had to discontinue NVP within 18 weeks after the switch because of HSR (2 rashes and 1 hepatotoxicity).
EfficacyAntiretroviral efficacy was studied for treatment-naive patients (n=1,080), treatment-experienced patients starting NVP having undetectable HIV-1 RNA (n=1,819), and treatment-experienced patients hav-ing detectable HIV-1 RNA (n=2,225). A total of 343 patients had a missing baseline CD4+ T-cell or viral load, preventing their inclusion in the efficacy anal-ysis. A total of 734 (13%) patients switched from a twice-daily NVP-based regimen to a once-daily regi-men after a median (IQR) of 172 weeks (84–262) of NVP treatment and 6.5% switched from a twice-daily to a once-daily NVP regimen within 18 weeks after NVP initiation. At the moment of the switch, HIV-1 RNA was undetectable in 82.2% of patients and their median (IQR) CD4+ T-cell count was 460 cells/mm3 (311–670).
At 96 weeks, 84% of treatment-naive patients using a twice-daily regimen had undetectable HIV-1 RNA as compared with 86% of those on a once-daily regimen (P=0.540). Kaplan–Meier analysis showed no differ-ences between the groups in time to become undetect-able (log-rank P=0.76; data not shown). Adjusted Cox models confirmed these results (twice daily versus once daily hazard ratio [HR] 0.97; P=0.71).
In treatment-experienced patients who started NVP with detectable HIV-1 RNA levels, NVP once daily was associated with a better virological outcome at week 96 as compared with patients on NVP twice-daily dosing schedule (71% versus 52% of patients with undetec-table HIV-1 RNA for the once-daily and twice-daily group, respectively; P 0.0001). Kaplan–Meier analysis of the TLOVR showed a significantly higher incidence of virological failure in the twice-daily group compared with the once-daily group (24.1% and 16.0% after 96 weeks of follow-up, respectively; log-rank P=0.006). However, the TLOVR was not significantly different in the adjusted Cox analysis, even after adjustment for
