ANATOMI SISTEM HEMATOLOGI Sistem hematologi tersusun atas darah dan
tempat darah diproduksi, termasuk sumsum tulang dan nodus limpa.
Darah merupakan medium transpor tubuh, volume darah sekitar 7%-10%
berat badan normal dan berjumlah sekitar 5 liter. Darah terdiri
dari atas 2 komponen utama, yaitu sebagai berikut. 1.Plasma darah,
bagian cair darah yang sebagian besar terdiri atas air,
elektrolit, dan protein darah. 2.Butir- butir darah (blood
corpuscles), yang terdiri dari komponen-komponen berikut ini.
Eritrosit : sel darah merah (SDM- red blood cell) Leukosit : sel
darah putih (SDP- white blood cell) Trombosit : butir pembeku darah
platelet. 2.2. STRUKTUR DAN FUNGSI NORMAL SEL DARAH PUTIH Pada
keadaan normal, darah manusia mengandung 4000 - 11.000 sel darah
seluruh jaringan tubuh manusia.
leukopenia. Macam-macam leukosit secara jelas meliputi :
1.Agranulosit.
mempunyai bintik-bintik sedikit kemerahan. Inti selnya bulat atau
panjang, warnanya lembayung muda. 2.Granulosit disebut juga
leukosit granular terdiri dari : a.Neutrofil atau polimorfonuklear
leukosit, mempunyai inti sel yang kadang-kadang seperti
terpisah-pisah, protoplasmanya banyak bintik-bintik halus/granula,
banyaknya 60%-70%. b.Eusinofil.
pertambahan jumlah sel darah putih (leukosit).
empat tipe utama leukemia : 1. LEUKEMIA MIELOGENUS AKUT (LMA)
stem hematopetik yang kelak berdiferensiasi ke semua sel mieloid;
monosit, granulosit (basofil, netrofil, eosinofil), eritrosit, dan
trombosit. Dikarakteristikan oleh produksi berlebihan dari
mieloblast. Semua kelompok usia dapat terkena; insidensi meningkat
sesuai dengan bertambahnya usia. Merupakan leukemia nonlimfositik
yang paling sering terjadi. 2. LEUKEMIA MIELOGENUS KRONIS (LMK)
keganasan sel stem mieloid.
bawah 20 tahun, namun insidensinya meningkat sesuai pertambahan
usia. Gambaran menonjol adalah : - adanya kromosom Philadelphia
pada sel sel darah. Ini adalah kromosom abnormal yang ditemukan
pada sel sel sumsum tulang. - Krisis Blast. Fase yang
dikarakteristik oleh proliferasi tiba-tiba dari jumlah besar
mieloblast.
LEUKEMIA LIMFOSITIK AKUT (LLA) Leukemia Limfositik Akut (LLA)
dianggap sebagai suatu proliferasi ganas limfoblas. Paling sering
Setelah usia
15 tahun , LLA jarang terjadi. 4. LEUKEMIA LIMFOSITIK KRONIS (LLK)
tahun. Negara-negara barat melaporkan penyakit ini sebagai leukemia
yang umum terjadi. LLK dikarakteristikan oleh proliferasi dari
diferensiasi limfosit yang baik (mudah dikenali sel-sel yang
menunjukkan jaringan asal). Kelompok Klasifikasi Leukemia Akut
Menurut French-American-British (FAB) Leukemia Limfositik Akut L-1
pada masa kanak-kanak: populasi sel homogen L-2 Leukemia limfositik
akut tampak pada orang dewasa: populasi sel heterogen L-3 Limfoma
Burkitt-tipe leukemia: sel-sel besar, populasi sel homogen.
Leukemia Mieloblastik Akut M-1 Diferensiasi granulositik tanpa
pematangan M-2 Diferensiasi granulositik disertai pematangan
menjadi stadium promielositik M-3 Diferensiasi granulositik
disertai promielosit hipergranular yang dikaitkan dengan pembekuan
intra vaskular tersebar (Disseminated intravascular coagulation).
M-4 Leukemia mielomonositik akut: kedua garis sel granulosit dan
monosit. M-5a Leukemia monositik akut : kurang berdiferesiasi M-5b
Leukemia monositik akut : berdiferensiasi baik M-6 Eritroblast
predominan disertai diseritropoiesis berat M-7 Leukemia
megakariositik. 2.5 ETIOLOGI Penyebab leukemia belum diketahui
secara pasti. Diperkirakan bukan penyebab tunggal tetapi gabungan
Genetik. Pengaruh genetik maupun faktor-faktor lingkungan
kelihatannya memainkan peranan , namun jarang terdapat leukemia
Kelainan Herediter.
Individu dengan kelainan kromosom, seperti Sindrom Down,
kelihatannya mempunyai insidensi leukemia akut 20 puluh kali lipat.
Faktor lingkungan. - Radiasi. Kontak dengan radiasi ionisasi
disertai manifestasi leukemia yang timbul bertahun-tahun kemudian.
- Zat Kimia. Zat kimia misalnya : benzen, arsen, kloramfenikol,
fenilbutazon, dan agen antineoplastik dikaitkan dengan frekuensi
yang meningkat khususnya agen-agen alkil. Kemungkinan leukemia
jaringan telah ditetapkan istilah HL-A (Human Leucocyte Lucos A).
Sistem HL-A individu ini diturunkan menurut hukum genetika sehingga
pertumbuhan sel darah normal. Proses patofisiologi leukemia dimulai
mengakibatkan penekanan hematopoesis normal, sehingga terjadi bone
menimbulkan organomegali, katabolisme sel meningkat, sehingga
terjadi keadaan hiperkatabolik.
kulit Efusi pleura kejang Leukemia Mielogenus Akut Kebanyakan tanda
dan gejala terjadi akibat berkurangnya produksi sel darah normal.
Peka terhadap infeksi akibat granulositopenia, kekurangan
granulosit Kelelahan dan kelemahan terjadi karena anemia
Kecendrungan perdarahan terjadi akibat trombositopenia, kurangnya
jumlah trombosit. Proliferase sel lukemi dalam organ mengakibatkan
berbagai gejala tambahan : nyeri akibat pembesaran limfa; sakit
Akibatnya:
Hematopoesis normal terhambat, mengakibatkan penurunan jumah
tetapi selalu terdapat sel imatur. Manifestasi infiltrasi leukemia
100/ml darah sangat mungkin terjadi infeksi sistemik. Disfungsi
imum mempertinggi resiko infeksi. Penghancuran sel besar-besaran
kemoterapi. Sering terjadi anoreksia, mual, muntah, diare, dan lesi
mukosa mulut. 2.10 PEMERIKSAAN DIAGNOSTIK a. Pemeriksaan
limfopoietik patologis sedangkan sistem lain terdesak (aplasia
sekunder).
Cairan Serebrospinal Bila terdapat peninggian jumlah sel patologis
remisi maupun keadaan kambuh.
jumlah kromosom seperti diploid (2n), hiploid (2n-a), hiperploid
(2n+a). Kariotip yang pseudodiploid pada kasus dengan jumlah
kromosom yang diploid. Bertambah atau hilangnya bagian kromosom
(partial depletion). Terdapatnya marker chromosome yaitu elemen
2.11 PENATALAKSANAAN
MEDIS DAN PENUNJANG a. Penetalaksanaan Medis Transfusi darah,
diberikan heparin Kortikosteroid (prednison, kortison, deksametason
dan sebagainya). Setelah dicapai remisi dosis dikurangi sedikit
demi sedikit dan akhirnya dihentikan. Sitostatika. Selain
obat lainnya. Umumnya sitostatika diberikan dalam kombinasi
bersama-sama dengan prednison. Pada pemberian obat-obatan ini
sering terdapat efek samping berupa alopesia (botak), stomatitis,
leukopenia, infeksi sekunder atau kandidiasis. Bila jumlah leukosit
kurang dari 2000/mm3 pemberiannya harus hati-hati. Infeksi sekunder
steril). Imunoterapi, merupakan cara pengobatan terbaru. Setelah
tercapai remisi dan jumlah sel leukemia cukup rendah (105-106),
imunoterapi mulai diberikan (mengenai cara pengobatan yang terbaru
masih dalam pengembangan). Cara pengobatan berbeda-beda pada setiap
klinik bergantung dari pengalaman, tetapi prnsipnya sama, yaitu
dengan pola dasar : 1. Induksi.
(sama seperti pasien kanker lainnya) maka pendekatan psikososial
kesehatan bagaimana cara mengatasi perdarahan hidung, misalnya
mencuci tangan, mandi 3x sehari. Menganjurkan keluarga klien untuk
menjaga keersihan diri mereka, membatasi jumlah pengunjung karena
dikhawatirkan dapat menularkan penyaki-penyakit seperti flu dan
batuk. Menciptakan lingkungan yang bersih dan jika perlu
pertahankan tehnik isolasi. 2.12 PROSES KEPERAWATAN Pasien Leukemia
a. Pengkajian 1. Riwayat pemajanan pada faktor-faktor pencetus,
seperti pemajanan pada dosis besar radiasi, riwayat infeksi virus,
genetik dan penyakit herediter. 2.
Pemeriksaan darah menunjukkan perubahan sel darah putih Anemia
penurunan berat badan, kelemahan dan kelelahan, pucat, malaise,
muntah dan anoreksia. Trombositopenia (jumlah trombosit rendah)
Petekia, Ekimosis, mudah memar, Kencenderungan perdarahan (pada
gusi) Netropenia Demam, berkeringat pada malam hari. 3. Infiltrasi
nyeri, respon pasien memberikan informasi tambahan tentang nyeri
klien. 3. Berikan dukungan emosional dan menentramkan kekuatiaran
pasien. Rasional : mengurangi ketakutan dan ansietas akibat
penyakit yang di derita. Ketakutan dan ansietas akan meningkatkan
persepsi nyeri. 4. Gunakan metode distraksi seperti relaksasi,
teknik pernapsan dalam, mendengarkan musik, dan imajinasi. Raional
neutropenia/ menurunnya sistem imun.
jarang, kadang-kadang,). Mengindikasi status gastrointestinal,
pernapasan, genitourinaria, dan imum dalam batas normal.
Menunjukkan higiene pribadi yang adekuat. Leukosit 4000 - 11.000/L,
Neutrofil : 150-300/L 36-37oC Intervensi Keperawatan : 1. Pantau
tanda / gejala infeksi (misalnya suhu tubuh, denyut jantung,
pembuangan, penampilan luka, sekresi, penampilan urin, suhu kulit,
lesi kulit, keletihan dan malaise, nilai leukosit). Rasional :
memberikan dasar untuk mengkaji perubahan jika terjadi kemungkinan
infeksi 2. Kaji faktor yang meningkatkan serangan infeksi
(misalnya: usia lanjut, tanggap imun rendah, malnutrisi). Rasional
perawatan diri : Aktivitas Kehidupan Sehari-hari (AKSI).
Istirahat jika mengalami keletihan
Melaporkan tingkat keletihan Hb : 13-16gr/dL (laki-laki), Hb :
12-14gr/dL (perempuan) Ht : lk = 40-58% Perempuan = 37-43%
ERITROSIT : Lk = 4,6-6,2 jt/mm3 Perempuan = 4,2-5,4 jt/mm3 HR
60-100x/mnt, RR 16-24x/mnt, TD 120/80mmHg, S :36-37oC Intervensi
Keperawatan : 1. Kaji Tanda-tanda Vital serta pantau respons
kardiorespirasi terhadap aktivitas (misalnya, takikardia,
disaritmia lain, dispnea, diaforesis, pucat, tekanan, hemodinamik,
6. Hindari obat-obat yang mengandung
aspirin. Rasional : karena aspirin mempengaruhi fungsi
trombosit.REPORT INTRODUCTION LEUKEMIA
THEORETICAL REVIEW
2.1 ANATOMY hematology systemHematology systems composed of
blood and blood spots were produced, including bone marrow and
lymph nodes. Blood is the transport medium of the body, the blood
volume of approximately 7% -10% of normal weight and amount to
about 5 liters. Blood is composed of the top two major components,
as follows.1.Plasma blood, the liquid part of blood that is
composed largely of water, electrolytes, and blood
proteins.2.Butir- grains of blood (blood corpuscles), which
consists of the following components.Erythrocyte: red blood cells
(red blood cell SDM-)Leukocytes: white blood cells (white blood
cell SDP-)Platelets: blood clotting grains - platelets.
2.2. STRUCTURE AND FUNCTION NORMAL WHITE BLOOD CELLSIn normal
circumstances, human blood contains 4000-11000 white blood cells
per microliter. Of these, the amount, the number of cells is most
granulocytes (leukocytes polimorfonukleus, PMN). Young granulocytes
cells have a nucleus shaped like a horse, which will be transformed
into a cell multilobular with age. Most of these cells contain
granules neutrophilic (neutrophils), but the fraction containing
granules which can be stained with acid dyes (eosinophils), and
partly contains basophilic granules (basophils). Two types of cells
commonly found in peripheral blood lymphocytes, which have a large
round nucleus and cytoplasm slightly, and monocytes, which contains
many cytoplasmic not berglanula and has a kidney-shaped core. The
cooperation of these cells causes the body to have a strong defense
system against tumor trending, viral infections, bacteria, and
parasites (Ganong, 2008).
White Blood Cells function is as a soldier is to kill and eat
the body of germs / bacteria that get into the network RES
(reticuloendothelial system), where breeding in the spleen and
lymph nodes; as transporting / carrying of fatty substances through
the intestinal wall into the blood vessels of the spleen continues.
Besides leukocytes are cells in the blood vessels is also found in
all tissues of the human body. In most diseases are caused by the
entry of germs / infections, the number of leukocytes in the blood
will be more than usual. This is due to leukocyte cells that
normally live in the lymph nodes, is now circulating in the blood
to defend the body against the disease. If the number of leukocytes
in the blood exceeds 11,000 / mm3 called leukocytosis and less than
4000mm3 called leukopenia.Various leukocytes are clearly
includes:1.Agranulosit. Leukocyte cell that does not have granules
in them, which consists of:a. Lymphocytes, a wide network of
leukocytes produced from RES and lymph nodes, there is a great
shape and there are small, there are granules in their cytoplasm
and large core, the amount of 20% -25% and function to kill and
feed on bacteria that enter the body tissues.b.Monosit. Most were
made in the red marrow, larger than lymphocytes, function as
phagocytes and the number of 34%. Under the microscope it appears
that protoplasmanya wide, slightly blue-gray spots have slightly
reddish. Cell nucleus round or long, the color mauve.2.Granulosit
called granular leukocytes consists of:a.Neutrofil or
polymorphonuclear leukocytes, have a cell nucleus which is
sometimes as separate, protoplasmanya many fine spots / granules,
the amount of 60% -70%.b.Eusinofil. The size and shape similar to
neutrophil granules in their cytoplasm but larger, the number of
24%.c. Basophils, this small cell of eusinofil but has a
regular-shaped core, in protoplasmanya granules are large. The
number of half red marrow, its function is unknown (Syaifuddin,
2006).
2.3 LEUKEMIA1. DEFINITIONSLeukemia initially described by
Virchow in 1847 as "white blood", is a neoplastic disease
characterized by the abnormal proliferation of hematopoietic cells
(Price, 1994).Leukemia is uncontrolled proliferase leukocytes in
the blood, bone marrow, and reticuloendothelial tissue (tuker,
1998).Leukemia is a disease characterized by excessive
proliferation of early (young cells) of white blood cells (WBC)
(engram, 1998).Leukemia is a neoplastic proliferative of white
blood cell precursor, which cause diffuse replacement of normal
bone marrow by leukemic cells with accumulation of abnormal cells
in the peripheral blood and infiltration of organs such as liver,
spleen, lymph nodes, meninges, and gonadal cells leukemi
(Underwood, 1999) ,Leukemia is the proliferation of irregular or
accumulation of white blood cells in the bone marrow, replace
normal bone marrow elements. Also been a proliferation in the
liver, spleen and lymph nodes and invasion nonhematologis organs,
such as the meninges, gastrointestinal tract, kidney and skin
(Smeltzer, 2001).Leukemia is a disease of the white blood cells
that undergo repeated division-ulang.penyakit this sort of cancer
that attacks white blood cells. As a result, the white blood cell
function is compromised, even the red blood cells can be forced
because of this excessive growth of red blood cell count decreased
(Irianto, 2004).Leukemia (blood cancer) is a disease characterized
in the number of white blood cells (leukocytes). This increase very
rapidly and uncontrollably and form of white blood cells is
abnormal (Orphans, 2003).Leukemia is a disease characterized by
excessive early proliferation of white blood cells (Hand, 2008)Some
understanding of the above it can be concluded that Leukemia is a
hematological system diseases characterized by excessive
proliferation and abnormal white blood cells resulting in impaired
function of white blood cells.
2.4 CLASSIFICATION OF LEUKEMIALeukemia can diklafikasikan
into:1. Maturity cell:Acute (cells of origin of poorly
differentiated)Chronic (more mature cells)2. Types of cell
originMyelocytic (Mieloblast generated bone marrow)Lymphocytic
(limfoblast produced by the lymphatic system)Normally, the cells of
origin (mieloblast and limfoblast) does not exist in the peripheral
blood. Maturity of cells and cell types are combined to form the
four main types of leukemia:1. MIELOGENUS ACUTE LEUKEMIA
(LMA)Mielogenus Acute Leukemia (LMA) or acute myelocytic leukemia
or can also be called acute granulocytic leukemia (LGA), the
hematopetik stem cells that later differentiate into all cells of
the myeloid; monocytes, granulocytes (basophils, neutrophils,
eosinophils), erythrocytes, and platelets. Characterized by
excessive production of mieloblast. All age groups can be affected;
the incidence increases with age. Is a non-lymphocytic leukemia is
the most common.
2. MIELOGENUS CHRONIC LEUKEMIA (CML)Mielogenus Chronic leukemia
(CML) or chronic myelocytic leukemia or chronic granulocytic
leukemia (LGK), also included in the stem cell myeloid
malignancies. However, there are more normal cells in the appeal in
the acute form, so the disease is milder. Genetic abnormality
called chromosomes of Philadelphia found 90% to 95% of patients
with CML. LMK rarely attack people under 20 years, but the
incidence increases according to age.Picture stand out are:- The
Philadelphia chromosome in the cells - blood cells. It is an
abnormal chromosome found in the cells - bone marrow cells.- Blast
Crisis. Phase is characterized by the sudden proliferation of a
large number mieloblast. This finding indicates the conversion of
CML into the LMA. Death often occurs within a few months when the
cells - leukemia cells become resistant to chemotherapy during the
blast crisis.3. acute lymphocytic leukemia (ALL)Acute lymphocytic
leukemia (ALL) is considered as a malignant proliferation of
lymphoblasts. Most often occurs in children, with men more than
women, with peak incidence at the age of 4 years. After age 15, LLA
rare.4. chronic lymphocytic leukemia (LLK)Chronic lymphocytic
leukemia (LLK) tends to be a mild disorder primarily affecting
individuals between the ages of 50 to 70 years. Western countries
reporting the disease is as common leukemia. LLK is characterized
by proliferation of lymphocytes good differentiation (easily
recognizable cells that indicates your home network).
According to the Classification of Acute Leukemia
GroupFrench-American-British (FAB)
Acute lymphocytic leukemiaL-1 in childhood: a homogeneous cell
populationL-2 acute lymphocytic leukemia seen in adults: a
heterogeneous cell populationL-3 Burkitt's lymphoma-leukemia types:
large cells, homogeneous cell population.Acute myeloblastic
leukemiaM-1 granulocytic differentiation without maturationM-2
granulocytic differentiation accompanied maturation into a stadium
promielositikM-3 granulocytic differentiation accompanied
promielosit hipergranular associated with intravascular clotting
spread (Disseminated intravascular coagulation).M-4 acute
myelomonocytic leukemia: both cell lines granulocytes and
monocytes.M-5a acute monocytic leukemia: less berdiferesiasiM-5b
acute monocytic leukemia: well-differentiatedM-6 Eritroblast
predominant accompanied by heavy diseritropoiesisM-7 Leukemia
megakariositik.
2.5 EtiologyThe cause of leukemia is not known with certainty.
It is estimated that not a single cause but a combination of risk
factors, among others:Infected with the virus. Viral agents has
long been identified as a cause of leukemia in animals. In 1980,
the HTLV-1 virus was isolated from a human T cell leukemia in a
patient with lymphoma lymphocytes skin and since then isolated from
serum samples of patients with T-cell leukemiaGenetic factors. The
influence of genetic and environmental factors seem to play a role,
but there are rare familial leukemia, but the incidence of leukemia
is higher than the siblings of children are affected, with the
incidence increasing to 20% in monozygotic twins
(identical).Hereditary disorders. Individuals with chromosomal
abnormalities, such as Down syndrome, seems to have acute leukemia
incidence 20-fold.Environmental factors.- Radiation. Contact with
ionizing radiation arising accompanied by manifestations of
leukemia many years later.- Chemicals. Chemical substances for
example: benzene, arsenic, chloramphenicol, phenylbutazone, and
antineoplastic agents is associated with increased frequency in
particular alkyl agencies. The possibility of leukemia increased in
patients treated with either radiation or chemotherapy.
2.6 PathophysiologyIf the cause leukemia virus, the virus will
get into the human body if the antigen structure in accordance with
the structure of the human antigen. When the antigen structure of
the individual is not the same as the structure of viral antigen,
the virus rejection as in other foreign objects. Human antigen
structure formed by the antigen structure of various organs,
especially the skin and mucous membranes are located on the surface
of the body (also called antigens skin tissue). By WHO to the
network has been established term antigen HL-A (Human Leukocyte
Lucos A). HL-A system of this individual is derived according to
the law so that the role of genetic factors in the etiology of race
and family leukemia can not be ignored.Leukemia is a proliferation
of cell maker systemic blood and is usually fatal. Leukemia said
blood disease caused by damage to the blood cells that
manufacturers bone marrow. The disease is often called a blood
cancer. The actual state of the bone marrow to work actively making
blood cells but produced is abnormal blood cells and these cells
urgent growth of normal blood cells.Leukemia pathophysiological
process starts from haematological stem cell malignant
transformation and its derivatives. Malignant proliferation of
these stem cells produce results in suppression of leukemia cells
and normal hematopoesis, resulting in bone marrow failure, leukemia
cell infiltration into the organs, causing organomegaly, increased
cell catabolism and therefore is hiperkatabolik.2.8 CLINICAL
FEATURESTypical symptoms of leukemia in
general:PaleHotSplenomegalyHepatomegalyLymphadenopathyBleeding may
be ecchymosis, petechiae, epitaksis, and bleeding gumsSymptoms that
are not typicalPain / joint pain or bone pain misinterpreted as
rheumatismPurpura lesions on the skinPleural
effusionconvulsions
Mielogenus Acute LeukemiaMost of the signs and symptoms caused
by reduced production of normal blood cells.Susceptible to
infection due to granulocytopenia, deficiency of
granulocytesFatigue and weakness occurs due to anemiaBleeding
tendency caused by thrombocytopenia, a lack of
platelets.Proliferase lukemi cells in the organ resulted in a wide
range of additional symptoms: pain due to enlargement of the
spleen; headache or vomiting as a result of meningeal leukemi
(common in lymphocytic leukemia); and bone pain due to spinal cord
deployment.
Mielogenus Chronic LeukemiaCML clinical picture similar to the
description of the LMA, but the signs and symptoms are milder. Many
patients who show signs and symptoms for many years. There is an
increase in leukocytes, sometimes up to an incredible amount. The
spleen is often enlarged.
Acute lymphocytic leukemiaBerploriferasi immature lymphocytes in
the bone marrow and peripheral tissues and interfere with normal
cell development. As a result:Normal hematopoesis hampered,
resulting in a decrease in the sheer number of leukocytes, red
blood cells, and platelets. Low amount of erythrocytes and
platelets and leucocytes amount can be low or high but there is
always the immature cells.Manifestations of leukemia infiltration
into other organs are more common in LLA than other types of
leukemia and resulted in:- Pain due to an enlarged liver and
spleen- Headache- Vomiting due to the involvement of meninges, and-
Bone pain.
Chronic lymphocytic leukemiaMost patients are asymptomatic and
diagnosed by the time a new physical handling or treatment for
other diseases. Manifestations that may occur
for:AnemiaInfectionEnlargement of the lymph nodes and abdominal
organsThe number of erythrocytes and platelets may be normal or
decreased.Decrease the number of lymphocytes (limfositopenia)2.9
COMPLICATIONSLeukemia complications include bleeding and infection,
which is the major cause of death. The formation of kidney stones,
anemia and problems gastroentestinal is another complication. The
risk of bleeding associated with deficient levels of platelets
(thrombocytopenia). Low platelet figures marked with bruises
(ecchymosis) and petechiae (bleeding spots reddish or grayish as a
needle in the skin surface). Patients also may experience heavy
bleeding if Jumah platelet count fell to below 20,000 / mm3 blood.
With no apparent reason, fever and infections can increase the
likelihood of bleeding.Because of the lack of mature granulocytes
and normal, the patient is always in a state of danger of
infection. The possibility of infection increases with the degree
of neutropenia, so if granulocytes below 100 / ml of blood is very
possible systemic infection. Imum dysfunction increases the risk of
infection.Massive cell destruction that occurred during the
administration of chemotherapy will increase uric acid levels and
make patients susceptible to the formation of kidney stones and
renal colic. Then the patient requires a high fluid intake to
prevent crystallization of uric acid stone
formation.Gastrointestinal problems can occur due to abnormal
leukocyte infiltration into the abdominal oran apart from the
toxicity of chemotherapy drugs. It often happens anorexia, nausea,
vomiting, diarrhea, and oral mucosal lesions.
2:10 DIAGNOSTIC EXAMINATIONa. Laboratory examinationSymptoms
seen in peripheral blood based on bone marrow disorders such as
pancytopenia, lymphocytosis which sometimes causes peripheral blood
picture menoton and there are blast cells. The presence of blast
cells in the peripheral blood is gajala patognomik to
leukemia.kolesterol may lower uric acid may increase,
hipogamaglobinea. From the examination of the bone marrow will be
found menoton picture, which only consists of limfopoietik
pathological cells whereas the other systems of urgency (aplasia
secondary). In the LMA besides menoton picture, seen also the
hiatus leukemia is a state that shows a lot of blast cells
(myeloblasts), some old cells (segments) and much less form-cell
maturation in between (promielosit, mielosit, metamielosit and stem
cells).
b. Spleen biopsyThis examination showed proliferase leukemia
cells and cells derived from spleen tissue is pressed, like normal
lymphocytes, RES, granulocytes, and pulp cells.c. Bone Marrow
punctureLumbar bone marrow is making a bit of bone marrow fluid,
which aims for an assessment of iron stores, getting specimens for
examination bakteriovirologis (microbiological culture), for
diagnosis sitomorfologi / evaluation of product origin maturation
of blood cells. The place is usually used to puncture bone marrow
aspiration is the posterior superior iliac spine (SIPS), iliac
crest, anterior superior iliac spine (SIAS), sternum between ribs
2nd and 3rd midsternal or slightly right (not more than 1 cm),
dorsal spine / spinous processes of the lumbar vertebrae.d.
Cerebrospinal fluidIf there is a pathological elevation of the
number of cells and proteins, means a meningeal leukemia. This
disorder can occur at any time in the course of the disease either
in a state of remission and relapse situation. To prevent given
methotrexate (MTX) in intrathecal routinely on each new patient or
a patient who showed symptoms of elevated intracranial pressure.e.
CytogenicIn the case of CML 70-90% show chromosomal abnormalities,
namely chromosome 21 (of Philadelphia chromosome or Ph 1). 50-70%
of the patients had abnormal LLA and LMA form:Abnormalities such as
diploid number of chromosomes (2n), hiploid (2n-a), hiperploid (2n
+ a).Karyotype which pseudodiploid the case with the diploid number
of chromosomes.Increases or the loss of part of chromosome (partial
depletion). The presence of the marker chromosome is the element
that is not morphologically normal chromosomes; from big sting
shape to very small. To determine the type of treatment should be
known abnormalities were found. In leukemia usually obtained from
the peripheral blood lymphocytosis form more than 80% or there are
blast cells. Also required an examination of the bone marrow by
using an electron microscope will look for pathological cells.
2:11 MEDICAL MANAGEMENT AND SUPPORTa. Medical
PenetalaksanaanBlood transfusions, usually given if hemoglobin
levels less than 6g%. In the severe thrombocytopenia and massive
bleeding, platelet transfusions may be administered and if there
are signs of DIC can be given heparinCorticosteroids (prednisone,
cortisone, dexamethasone, and so on). Having achieved remission
dose gradually reduced and finally stopped.Sitostatika. Besides
sitostatika long (6-mercaptopurine or 6-MP, methotrexate, or MTX)
at this time is also used new and more potent as vincristine
(Oncovin), rubidomisin (daunorubycine) and various other drug
names. Sitostatika generally given in combination with prednisone
together. In the administration of these drugs often there are side
effects such as alopecia (balding), stomatitis, leukopenia,
secondary infection or candidiasis. If leukocyte counts less than
2000 / mm3 gift must be careful.Secondary infection avoided (better
patients treated at room sterile / sterile).Immunotherapy, the new
treatments. Having achieved remission and the leukemia cell count
is low enough (105-106), immunotherapy began to be given (the
latest method of treatment is still in development).
Different ways of treatment at each clinic relies on experience,
but prnsipnya the same, namely the basic pattern:1. Induction.
Intended to achieve remission with various drugs to blast cells in
the bone marrow is less than 5%.2. Consolidation. Intended that the
remaining cells are not rapidly reproduce themselves again.3.
Rumat. To maintain a longer period of remission order. Usually by
giving sitostatika half the usual dose.4. Reinduksi. Dimaksukan to
prevent relapse. Usually done every 3-6 months with the
administration of drugs such as on induction for 10-14 days.5.
Prevent the occurrence of leukemia in the central nervous system.
Given by intrathecal MTX and cranial radiation.6. Treatment of
immunologic.
b. Nursing ManagementPatient problems that need to be generally
the same as other patients suffering from blood diseases. But
because the prognosis of patients are generally less encouraging
(the same as other cancer patients), the psychosocial approach
should be preferred. Which should be endeavored is the room that
aseptic and aseptic way of working that well. The attitude of
nurses were friendly and gentle expected not only to patients but
also to the families in this very sensitive feelings if knowing his
illness or his family. Some of the ways that we can recommend is to
avoid brushing your teeth too hard, because toothbrush bristles can
injure the gums. Advises clients to be careful when walking on
slippery floors as a bathroom to keep from falling. Provide client
and family health education how to cope with bleeding nose, for
example dammed with cotton or gauze, head thrown back position. To
handle clients infection should maintain personal hygiene, such as
washing hands, bathing 3x daily. Advise the client's family to keep
keersihan themselves, limiting the number of visitors because it
feared could transmit illnesses such as flu-disease and cough.
Creating an environment that is clean and if necessary maintain
isolation techniques.
NURSING PROCESS 2:12 Leukemia Patientsa. Assessment1. History of
exposure to trigger factors, such as exposure to large doses of
radiation, a history of viral infection, genetic and hereditary
diseases.2. Physical examination may show
manifestations:Enlargement of the bone marrow with leukemic cells
were further suppress bone marrow function, causing some of the
symptoms below:HeadacheInfectionBlood tests showed changes in white
blood cells anemia weight loss, weakness and fatigue, pallor,
malaise, vomiting and anorexia.Thrombocytopenia (low platelet
count) petechiae, ecchymoses, easy bruising, bleeding tendency (of
the gums) neutropenia fever, sweating at night.3. Infiltration of
other organs with leukemia cells that cause some symptoms such
as:HepatomegalySplenomegalyLymphadenopathyNyri bones and jointsGum
hypertrophy.b. Nursing Diagnosis and InterventionNursing diagnoses
that may arise include:1. Pain associated with infiltration of
leukocytes into tissue systemicObjectives: after the act of nursing
on the client pain will be eased.Criteria Results:Expressed pain
decreased with indicators 1-3 (none, mild, moderate)Calm facial
expression. There is no non-verbal instructions about the painHR

medication as prescribedTaking an active role in providing
analgesic.1-3 pain scale (none, mild, moderate)
Nursing Intervention:1. Assess the characteristics of pain:
location, quality, frequency, and duration.Rationale: To provide a
basis for assessing changes in levels of pain and evaluate
interventions.2. Give analgesic therapy in accordance with the
doctor's instructions. Assess the patient's response to the
analgetikRational: analgesic is a pharmacological agent serves to
reduce pain, analgesics tend to be more effective when given early
in the cycle of pain, the patient's response provide additional
information about the client's pain.3. Provide emotional support
and reassuring kekuatiaran patient.Rational: reducing fear and
anxiety due to illness in the suffering. Fear and anxiety will
increase the perception of pain.4. Use distraction methods such as
relaxation techniques pernapsan in, listening to music, and
imagination.Raional: diversion or distraction techniques can make
reduce the perceived pain patients because patients do not focus on
the pain they experienced.2. The risk of infection associated with
immune deficiencies associated with neutropenia / decrease the
immune system. Objectives: after nursing actions on the client, the
client will be free of infection symptoms.Criteria Results:Risk
factors will disappear indicated by the patient's immune
statusPatients showed risk control, evidenced by the following
indicators (between 1-3: never, rarely, sometimes,).Indicate the
status of gastrointestinal, respiratory, genitourinary, and
immunity within normal limits.Demonstrate adequate personal
hygiene.Leukocytes 4000-11000 / L, Neutrophils: 150-300 /

infection (eg, body temperature, heart rate, disposal, appearance
of the wound, secretion, urine appearance, skin temperature, skin
lesions, fatigue and malaise, the value of leukocytes).Rational:
provide a basis for assessing the change in case of the possibility
of infection2. Assess factors that increase invasive infections
(eg, the elderly, the immune response is low,
malnutrition).Rational: to determine next intervention3. Instruct
to maintain personal hygiene to protect the body against infection
in patients and families.Rational: personal hygiene can protect the
body in order to minimize exposure to infective organisms.4. Give
antibiotic therapy when necessary in accordance with the doctor's
instructions.Rational: given as a prophylactic or treat specific
infections5. Maintain isolation techniques, if necessary.Rational:
The rooms were insulated to minimize the exposure of the patient
from the source of the infection.6. Protect the patient from
cross-contamination by not assigning the same nurse for every
patient infection and separates patient infection in different
rooms.Rational: cross-contamination can increase the risk of
infection to the client.
3. Activity intolerance: the overall weakness due to
anemia. Objectives: after the act of nursing at the client, there is
increased activity tolerance.Criteria Results:Mentolenrasi regular
activity and shown with durability, energy savings, and self-care:
Activities of Daily Living (SCAI).Show energy savings,
characterized by indicator 1-5 (not at all, mild, moderate, severe,
or very severe), aware of the limitations of energy, balancing
activity and rest.Verbalize understanding of the need for oxygen,
medication, and / or equipment that could increase tolerance to
activity.Break if subjected to fatigueReported levels of fatigueHb:

Nursing Intervention:1. Assess and Vital Signs monitor
cardiorespiratory response to the activity (eg, tachycardia,
another disaritmia, dyspnea, diaphoresis, pallor, pressure,
hemodynamic, and respiratory frequency) patients and hemoglobin
concentration in the blood.Rational: provide the basis for
determining the level of intervention and client capabilities2.
Evaluation reports weakness, note the ability to participate in
daily activities.Rational: inability to determine the degree and
effect.3. Provide a quiet environment and need to rest without
interruption.Rational: save energy for activity and cellular
regeneration or network connection.4. Monitor the intake of
nutrients to ensure the adequacy of energy sources as well as
provide input adequate protein and calories.Rational: calories and
nutrients of protein is quite can help restore lost energy and
improve exercise tolerance.5. Teach setting activities and time
management techniques to prevent fatigue.Rational: setting activity
and time management can regulate the use of energy so as to prevent
fatigue.
4. Risk of injury: bleeding associated with
thrombocytopeniaObjectives: after nursing actions on the client,
showing the risk of injury decreases.Criteria Results:Shows the
risk control is evidenced by this indicator 1-3 (never, rarely,
sometimes).Avoid physical injury.Prepare a safe environment (for
example, negating the irregularity and spills, the placement of
handrails, use rubber mats, as well as handrails in the
bathroom).Signs of bleeding is reduced. Ecchymosis no / reduced,
